Patients and families with exceptional needs

When a patient or family need arises, which isn't covered by the Medicare Hospice Benefit, The InFinity Care Foundation is there to provide the funding needed.

Those needs can be as important as a telephone bill being paid, so family members can keep in touch; or as extended as a last trip to visit a dying loved one.

To access the Foundation funding for exceptional patient needs, the hospice Interdisciplinary Team (IDT), initially evaluates, completes a "Funding Request Form" and a "Financial Assessment Form". Following IDT approval, the request will be forwarded to the Administrator, who will present it to the Foundations' Chairman. The Foundation's Board of Advisors then evaluates, and will base their decision to fund on the merit of each request.
