Long Term Care and Hospice can and should work together in providing care needed at the end of life for residents who sometimes call "home":

  • the Nursing Home,
  • Assisted Living Facilities,
  • or, Independent Living facilities.

The Long Term Care Facilities we work alongside in caring for residents, will know we feel privileged to be included as professionals in delivery of hospice services to patients and their families.

InFinity Care of Tulsa is committed to bringing to these partnerships the following:

  • Excellence in patient and family care. As stated in our service commitment, "we provide the right care for every patient, every time."
  • Coordination of care planning which removes any opportunity for error or omission of details related to the palliative plan of care and it's interface with the LTCF's plan of care.
  • Improvement of MDS evaluation and appropriate coding for the hospice patient, related to case mix and reimbursement.
  • Hospice Staff oriented to the LTCF regulations and standards.

If you are the Administrator, DON, Medical Director or other discipline in the LTCF setting, InFinity Care of Tulsa is NOT "just another" hospice. We are truly "consultants" in end of life care, providing hospice staff experienced in LTC, a Medical Director with palliative care training and experience and a leadership team with the commitment to stand behind our partnerships.
